Principle
CoolSculpting selectively kills fat cells by cooling them and is a localized treatment for reducing fat without surgery.
The treatment was announced in 2008 as Selective Cryolipolysis. Its effects in reducing fat were recognized, and it was approved by the FDA (US Food and Drug Administration) in 2010. In 2017 it was approved by the Ministry of Health, Labour and Welfare as a “device for reducing fat by cooling”.
Selective Cryolipolysis utilizes the fact that the nature of fat cells makes them more susceptible to low temperatures than other tissue. Under the right conditions, it freezes (crystallizes) only the fat, selectively killing the fat cells without damaging the surrounding tissue including skin, blood vessels, and nerves.
Fat is stored inside fat cells. When fat freezes (crystallizes), the fat cell undergoes a natural death (apoptosis). The fat cells are then gradually removed together with the fat they are storing by white blood cell phagocytosis. (The cells are consumed by white blood cells.) Because the fat cells are consumed gradually over several months, the fat is removed safely without any abnormal increase of lipid in the blood.
* This cell death occurs by a natural biological process, unlike death which produces inflammation or other effects in the surrounding area.

Examination and counseling
At the AOHAL CLINIC, we will check the patient’s fat conditions from a variety of angles while asking about the patient’s preferences. Because fat and its conditions vary from person to person, we will examine the body by sight and by touch from all directions. CoolSculpting is treatment intended for body styling, not for weight loss. It is not the same as reducing weight by means of exercise and diet controls. Because the purpose of CoolSculpting is to reduce fat cells, we will ask about your ideal body style and design effective locations of treatment. We will then propose treatment locations and a number of treatments to help you move closer to your ideal body.

Treatment
At AOHAL CLINIC, patients receive treatment in completely private rooms. The treatment time varies according to the individual, however it generally lasts from several 10s of minutes to several hours. After the nurse attaches CoolSculpting to the treatment location, the patient is left alone to spend the time as he or she wants. Patients may talk on mobile phones, or bring in a PC to watch movies on. Because the treatment does not produce any strong pain while it is in progress, many patients nap. However patients must refrain from any large movements that would cause CoolSculpting to come off of the treatment location.

After treatment
The nurse removes CoolSculpting and massages the treatment location. The patient then changes clothes and returns home. Because CoolSculpting is not surgery, many patients return to work or go out for fun immediately after treatment.
Can be combined with other procedures for more effective treatment.
CoolSculpting is thought to be most effective against soft subcutaneous fat.
For hard subcutaneous fat that is bad for circulation and cellulite fat, we recommend that three-dimensional suctioning and pressing in order to soften the fat before treatment, and we also recommend combining CoolSculpting with deep heating treatment that boosts fat metabolism.
The treatment combinations can also be used to combat subcutaneous fat and visceral fat, and to improve overall body metabolism.

Risks associated with the treatment
・Effects of the treatment vary depending on the individual. Depending on the conditions, there are cases when no improvement is seen.
・During treatment, the patient may experience sensations of pulling, being grabbed, coolness, or pain.
These sensations will disappear as the treatment location becomes numbed with increasing treatment time.
・The ordinary side-effects occurring after treatment include temporary redness, swelling, internal bleeding, tingling or prickling feeling, itchiness, pain, desensitization, and hypersensitivity. These symptoms will disappear over a period of several days to several weeks.
